National Health Insurance
After turning in your Alien Registration application, head over to the National Health Insurance Desk. Here, turn in the following documents:
- Two national health insurance application forms - the Kokumin Kenko Hokenzei Shinkokusho and the Kokumin Kenko Heoken Ido Todoke.
- Passport
- Receipt of Alien Registration Application, which you will get after applying for the ARC.
The National Health Insurance card and payment forms will be mailed to you about a month later. Keep your national health insurance card with you. You will need it to see the doctor. Also, in case you get into an accident, you will have proof of insurance. Hospitals here will withold treatment until they can determine that you have insurance and are able of paying. If you are unconcious, that's too bad for you.
The payment forms are due every month. It costs about 1,000 yen a month. Bring the form to your local SunKus or 7-11 to pay.
